  For new families only:

  Included in your fall packet you will find the Tuition Express Form (weekly billing), NELC half-sheet Emergency Card, Admission and Emergency Contact Form, Permission Form, Health Care Summary (must be completed by a DOCTOR), Immunization Record, Family Profile, Toilet Training Policy (Preschoolers only), Allergy Forms if needed and FOB Form (your access key card to get in the building).  These forms, along with the FOB payment, must be completed and returned prior to your child (ren’s) start date.
   

   

  Please Note: IF YOUR CHILD HAS FOOD ALLERGIES, ASTHMA, DIABETES, OR SEIZURES YOU MUST FILL OUT ADDITIONAL FORMS BEFORE YOUR CHILD CAN START AT THE NELC, ESPECIALLY IF THEY REQUIRE TO HAVE MEDICATION ON SITE.  **THESE FORMS WILL BE INCLUDED IN YOUR PACKET. **
